My aunt is 55 years old and she was diagnosed with heart failure(40%) since August 2014. 2 months back, she has been diagnosed with renal failure and she is on regular hemodialysis, 3 times a week. Is kidney transplant possible or she has to depend on dialysis for the rest of her life?

If your aunt has severe heart disease, this may complicate any attempt to perform a kidney transplant.  In some cases, combined kidney and heart transplant can be done, but these are very rare.  I suggest that your aunt consult with her nephrologist to see if she is a transplant candidate and ask to be evaluated by a transplant center.
